Section A: Family Contact Information

For purposes of this form, Adult 1 refers to the person filling out the form (usually a parent). Adult 2 is used when there is a second adult (usually a parent, step-parent, grandparent or guardian) with whom you share primary responsibility for your child.

If there are two contacts, all emails and mailings will go to both contacts. In case of urgent need, the person filling out the form will be notified first.

Section B: Student Enrollment Information
Student 1 Information
If your child does not have a Hebrew name, enter "None." Should you like to have a naming ceremony for your child, please contact Rabbi Marc Israel at rabbi@tikvatisrael.org.
Netivot tuition is $750/child for the year ($800 after May 3rd) plus a $50 registration fee. We believe that financial limitations should not get in the way of a child receiving a Jewish education. We ask that every family commits to some financial contribution; however we will discuss financial aid options for any family who needs it. If you select yes, you will discuss your payment ability in a confidential meeting with Rabbi Israel.
This is the same as your child's grade for the 2024-2025 secular school year unless you have made other arrangements with Rabbi Israel.

Learning Commitment

At Tikvat Israel Congregation, we are committed to lifelong Jewish learning. The combined Kindergarten-2nd Grade class will meet every Sunday, except for holidays, from 9:30- 11 am at Tikvat Israel. We expect parents/guardians to assist their children in getting the most out of this joyful Jewish learning experience by making every effort to have your child(ren) arrive on time and to stay for the entire 1.5 hour class each week. We do not expect perfect attendance; however, too many absences can be detrimental to the whole class. Learning opportunities will be available for parents throughout the school year to enhance parent/guardian Jewish knowledge and give parent/guardians the tools to incorporate into their home life what each child is learning in the classroom.

By checking this box, I confirm I have read, understand and agree to the above.
Media Release

From time to time your child’s photo may be taken in our classrooms or special events.  We use these photos in the synagogue newsletter, on our synagogue display boards, the Tikvat Israel website, social media and promotional items.

Section D: Congregation Community

The work of raising and educating Jewish children cannot be done in the school alone, but is a project of the school, the family and the community.  When you come in the building and spend time learning and volunteering with us, you make a powerful statement to your children that Jewish learning is a lifelong process and that we are all on the same team in creating these experiences for them.

Section E: Payment Information

Parents/guardians and Tikvat Israel work in financial partnership to cover the costs of running a religious school. (Tuition covers approximately 75% of the costs of the school.)
